What Is CHADIS?

Here’s why CHADIS is important, how we use it, and what this means for you as a patient.
We want to start off by saying, CHADIS isn’t for every patient. Most CHADIS Questionnaires only apply if your child has reached these stages.
15 months – At your 15-month appointment, Parent or Guardian will get an ASQ (Ages & Stages Questionnaire) sent to them through email to fill out before the appointment.
2 years & up – Parent or Guardian will be assigned a Developmental Screening Questionnaire through email to fill out before the appointment.
12 years & up – Patient will be assigned an adolescent mental health questionnaire to fill out in office.
Newborn through 6 months – Moms will be assigned a prenatal screening, done in office.
Others – There may be additional surveys in certain circumstances. If your child requires a CHADIS Questionnaire, we will contact you well in advance of your appointment to notify you.

What Is CHADIS and Why Is It Important?

CHADIS stands for Child Health and Development Interactive System. It is a secure online platform for pediatricians to track developmental, behavioral, and medical information from parents, teens, and teachers. This helps pediatricians make important clinical decisions for your child, as they continue to grow and regularly see their physician.
CHADIS contains questionnaires to help identify certain conditions.
These questionnaires are important, because they help physicians understand outside context, not given in office. This helps physicians make accurate, data-based decisions to make important medical decisions, to keep your child’s overall care, growth, and wellbeing on-track.

Why Do We Use CHADIS?

Priority Pediatrics uses CHADIS so we can give each patient more personalized care, while staying fully HIPAA compliant. Because CHADIS is fully HIPAA compliant, no one, other than Priority Pediatrics has access to your information. And, the other benefit of CHADIS, is by having parents or guardians complete their surveys before the appointment, our doctors have immediate access to their results during the appointment. Which means we can spend more time with each patient, to assure that their specific concerns are addressed.
